Responsibilities
- Assist patients in recovery and managing chronic conditions
- Provide case management
- Travel to assigned cases
Qualifications & Skills
- Bachelor's, Master's, or Doctoral Degree in Physical Therapy from an accredited institution
- Active state license
- BLS certification (may be required)
- At least 1 year of home care and case management experience
Key Duties
- Work during day shifts
- Participate in weekend rotations every six weeks
- Travel as needed
- Deliver patient care in diverse settings while complying with clinical protocols
