Role & Responsibilities
- Assess and treat adult patients in a skilled nursing setting to improve mobility and independence.
- Develop individualized care plans, evaluate movement dysfunction, implement evidence-based interventions, and document progress.
Qualifications & Skills
- Graduation from an accredited physical therapy program and active Minnesota PT license required.
- Experience in skilled nursing or rehabilitation settings is preferred.
- Must have at least one professional reference.
Key Duties
- Provide skilled physical therapy services, evaluate patient needs, and contribute to patient recovery efforts.
