American Traveler is seeking a travel Outpatient Physical Therapist for a 26-week assignment in Surprise, Arizona.

Responsibilities

• Work in an outpatient physical therapy clinic setting in Surprise, Arizona
• Provide services using InSync as the electronic medical record (EMR) platform
• Adhere to all relevant laws, regulations, and standards of practice

Requirements

• Active Physical Therapist license required; AZ compact license accepted to start, but full state license must be obtained during the assignment
• One professional reference required for consideration
• Social Security Number (SSN) required for consideration

Qualifications

• Must have previous work experience at this facility in a PRN or permanent capacity, separated for at least 1 calendar year to be eligible as a traveler

Pay

Pay package is based on 8 hour shifts and 40 hours per week (subject to confirmation) with a tax-free stipend amount to be determined.

Shift: 8 hours per day, 5 days per week
