Outpatient (OP) Physical Therapist position in South Bend, Indiana. This role involves evaluating all assigned patients, developing, implementing, and modifying treatment plans in accordance with patients' needs, goals, and physician directions. The therapist will delegate and oversee all discipline-specific therapy services, ensuring the design of functional treatment programs to facilitate effective patient return to community activities. The role emphasizes patient and staff education, adherence to professional standards, safety protocols, and organizational core values.

Responsibilities

• Complete comprehensive evaluations of assigned patients.
• Develop individualized treatment plans focusing on patient progress within established length of stay.
• Document medical records accurately and timely in compliance with all applicable guidelines.
• Participate in student programs and provide positive supervisory experiences.
• Pursue ongoing professional development and licensure renewal.
• Adhere to all regulatory standards and organizational performance improvement initiatives.
• Maintain ethical conduct and professional behavior.

Requirements

• Degree in Physical Therapy from an accredited school.
• 1+ year of outpatient clinical experience.
• CPR certification.

Schedule

• Days shift, 8 hours per day, minimum of 40 hours per week.
