Travel Physical Therapist – Long Term Care/SNF - $2,070 per week

About the role

CoreMedical Group is seeking a travel Skilled Nursing Facility (SNF) Physical Therapist for a 13-week assignment starting July 10, 2026, in Manchester, New Hampshire.

Responsibilities

• Provide physical therapy services to patients in SNF settings
• Develop and implement individualized treatment plans
• Evaluate patient progress and adjust treatment plans as necessary
• Document patient care activities and progress in medical records
• Collaborate with interdisciplinary teams to ensure comprehensive patient care

Requirements

• Current licensure as a Physical Therapist in the state of New Hampshire
• At least one year of recent relevant experience in SNF or similar setting
• Knowledge of Medicare and Medicaid regulations
• Excellent communication and interpersonal skills

Qualifications

• Master’s degree in Physical Therapy from an accredited program
• Passing scores on the National Physical Therapy Examination (NPTE)

Pay
