The therapist will work in a home health setting managing treatment plans to promote positive outcomes for patients with diverse diagnoses. Responsibilities

• Manage patient treatment plans to promote positive outcomes for diverse diagnoses in a home health setting.

Requirements

• One year of recent experience in Home Health is preferred.

Qualifications

• Current BLS certification (AHA preferred).
• Active state license.
• Supervisory professional references.
• Must complete drug screen and background screen.
