Travel Physical Therapist - Home Health

Travel Physical Therapist specializing in Home Health, providing outpatient therapy services in patients' homes. Join a supportive team focused on your growth and wellbeing, delivering meaningful patient care in a flexible travel role.

Responsibilities

• Conduct regular patient visits (5-6 per day)
• Perform assessments such as start of care, recertification, and resumption of care
• Manage patient care within a 50-mile radius
• No clinical involvement beyond assessments and therapy, with remote support from clinical coordinators

Qualifications & Skills

• Minimum of 1 year of experience
• ND Physical Therapy License
• BLS certification
• Experience or training in Home Health and OASIS
• Valid Driver's License and Auto Insurance required
• Ability to work independently in a home setting, managing patient care efficiently

Schedule

40 hours/week, Monday through Friday, 8-hour days, no weekends or holidays, with a short orientation period.

Scrubs of black, navy blue, or grey are required.
