About the role
Travel Home Health Physical Therapist responsible for providing patient care in a home health setting across multiple towns in VT and NH.
Responsibilities
- Screen, evaluate, re-evaluate, and treat patients
- Develop and implement treatment plans
- Document evaluations and goals
- Educate patients and families
- Coordinate with an interdisciplinary care team
- Maintain independence in patient visits
Schedule & Work Environment
Full-time, day shift, 40 hours per week, working 8-hour days in a home health setting.
Qualifications & Skills
- BS in Physical Therapy
- Current BLS certification
- At least 1 year of home healthcare PT experience
- Licensure in VT and NH or unencumbered compact license
- Preferred certifications include ASHA, CHT, or ACP
