Travel Physical Therapist - Home Health - $2,831 per week

About the role

Home Health Physical Therapist assignment in Concord, California serving a geriatric patient population. Average caseload of 5‑7 patients per day, day shift schedule (5×8 hours, 8:00 AM‑4:30 PM), no weekend or on‑call requirements. Territory covers up to a 60‑mile radius from the hub office. Assignment uses the HCHB EMR system; an iPad is provided for documentation. Mileage is reimbursed at the IRS standard rate.

Responsibilities

• Provide physical therapy services to home health patients.
• Document patient encounters using HCHB EMR (iPad provided).
• Maintain weekly productivity point requirements to qualify for guaranteed hours.
• Ensure timely communication and documentation.

Requirements

• Minimum 1 year of recent home health experience.
• Current BLS certification.
• Oasis experience.
• HCHB EMR experience (preferred).
• California PT license (if applicable).

Pay
