Responsibilities

• Deliver physical therapy services in a home setting
• Document care accurately and promptly via electronic health records
• Collaborate with patients, families, physicians, and multidisciplinary teams to develop and implement care plans
• Coordinate care with the interdisciplinary team

Requirements

• Massachusetts Physical Therapist Professional license
• At least 1 year of home care and travel experience
• Ability to travel to assigned cases
