Travel Physical Therapist Home Health - $2,690 per week

Physical Therapist (PT) specializing in Home Health in a day shift setting. This role involves providing outpatient therapy services to geriatric patients in the designated territory. Responsibilities

• Provide outpatient therapy services to geriatric patients in the designated territory
• Perform assessments and manage post-op surgeries, spinal conditions, and neurological disorders
• Complete timely documentation
• Maintain flexibility with territory
• Adhere to clinical standards to ensure quality patient care
• Use Homecare Homebase (HCHB) electronic charting system
• Complete OASIS documentation for staff completing Start of Care

Schedule

Days, 8 hours per shift, minimum of 40 hours per week. Orientation will be held on the upcoming Monday. Float rotation may be required.

Requirements

• Minimum of 1 year of relevant experience
• BLS certification
• Active license in hand
• Proficiency in Homecare Homebase (HCHB) electronic charting system
• OASIS experience required for staff completing Start of Care
• No first-time travelers
• Recent experience in outpatient, rehabilitation hospital, or skilled nursing facility settings
• Valid driver's license and reliable personal transportation
• Strong communication and documentation skills
• Traveler must be flexible with the territory
• Travel pairs are accepted with each assigned their own territory and transportation
