Responsibilities

• Deliver therapeutic services to adult and geriatric patients in a home health setting
• Emphasize rehabilitation and functional improvement
• Travel between patient visits within a radius of approximately 60 miles per day
• Document patient care using electronic systems including EPIC and Rover
• Adhere to home health care regulations and OASIS requirements

Requirements

• BLS certification
• Valid state license (including compact license accepted)
• At least 1 year of skilled home health experience
• Current COVID-19 vaccination series plus one booster
• Reliable transportation

Schedule

Dayshift, 8 hours per day, minimum of 40 hours per week.
