About the role

As a Physical Therapist at Visiting Nurse and Hospice for Vermont and New Hampshire, you will have the opportunity to create meaningful relationships with your patients while being an integral part of their care team. Your role will involve conducting evaluations, developing treatment plans, and providing therapy in a home health setting across the White River Junction, VT; Lebanon, NH; Bradford, VT; and Windsor, VT areas. This is a 13-week contract position with a day shift schedule.

Responsibilities

• Evaluate the patient's need for physical therapy and develop an appropriate treatment plan.
• Carry out treatment programs according to agency policies and professional standards of care.
• Document evaluations, treatment goals, and objectives accurately.
• Communicate effectively with all members of the healthcare team.
• Teach patients and family members the rationale for treatments and procedures.

Requirements

• Bachelor of Science in Physical Therapy.
• Must be a licensed Physical Therapist in VT and NH, or hold an unencumbered compact license.
• One year of experience as a PT in a home healthcare setting.

Pay

Pay package is based on 10-hour shifts and 40 hours per week (subject to confirmation) with a tax-free stipend amount to be determined. Contract and local rates may vary based on location and applicant residency.
