Travel Physical Therapist Home Health - $2,300 per week

About the role

Physical Therapist Home Health in a home health setting. This role involves providing outpatient physical therapy services to patients in their homes, focusing on patient rehabilitation and mobility. The position offers a short-term assignment with flexible training and orientation.

Responsibilities

• Conduct regular visits, including start of care, recertification, and resumption of care.
• Typical workload involves 5-6 visits per day, with the possibility of 3 additional visits if performing an admission assessment.
• Perform safety assessments and patient education during visits.
• Short orientation with a preceptor for 1-2 weeks will be provided.

Requirements

• BLS certification
• New Jersey Physical Therapy License
• Valid driver's license
• Auto insurance
• Minimum 1 year of experience required
• Prior home health experience preferred
• OASIS training preferred

Schedule

Monday through Friday, 8:00 AM to 5:00 PM, 40 hours per week (8‑hour day shifts).
