The role involves providing physical therapy services to patients, including start of care, recertifications, evaluations, and regular visits. The therapist will work in a patient-centered environment, supporting recovery and functional improvement through clinical expertise and compassionate care. Job Details Position involves working in a home health care setting with a focus on PT services. Schedule: Dayshift, 8-hour shifts, Monday through Friday, with rotating weekends, and some holiday rotation. The position requires a license in hand upon submission, with certification in Basic Life Support (BLS) from the American Heart Association. Minimum of 1 year of experience in home health care or related fields preferred; new graduates are not required but experience is beneficial. Responsibilities include performing start of care, recertifications, evaluations, and patient visits, with around 6.5 visits per day or week. Experience with common diagnoses such as CHF, orthopedic conditions, pneumonia, and the ability to manage patients accordingly. The candidate must have home care and home health experience, and access to a clinical manager or educator 24/7 or during business hours for support. Documentation will be completed using Homecare Homebase (HCHB) charting system. Additional requirements include scrubs or business casual attire and participation in a 2-week orientation with a preceptor. No clinical involvement, with profiles sent directly to the manager for phone interview.
