Physical Therapist Home Health (PT HH). This position is based in a home health care setting. The role involves providing physical therapy services to patients, including start of care, recertifications, evaluations, and regular visits. Job Details

• Setting: Home health care
• Schedule: Dayshift, 8-hour shifts, Monday through Friday, with rotating weekends and some holiday rotation
• License required in hand upon submission: Physical Therapist license
• Certification required: Basic Life Support (BLS) from the American Heart Association
• Experience: Minimum of 1 year in home health care or related fields preferred; new graduates are not required but experience is beneficial
• Typical workload: Around 6.5 visits per day or week
• Common diagnoses: CHF, orthopedic conditions, pneumonia; must be able to manage patients accordingly
• Home care and home health experience required
• Support: Access to a clinical manager or educator 24/7 or during business hours
• Charting system: Homecare Homebase (HCHB)
• Attire: Scrubs or business casual
• Orientation: 2-week orientation with a preceptor
• Interview: Profiles sent directly to the manager for phone interview; no clinical involvement required for the interview process
