About the role

We are seeking an experienced and compassionate Travel Physical Therapist (PT) to join our rehabilitation team for a 14-week contract assignment in a skilled nursing and long-term care setting. The ideal candidate will be committed to helping patients improve mobility, strength, balance, functional independence, and overall quality of life. This position offers a 36-hour weekly schedule and provides an excellent opportunity to gain valuable experience working with a diverse resident population in a supportive environment.

Responsibilities

  • Perform comprehensive physical therapy evaluations and assessments of residents.
  • Develop individualized treatment plans based on each resident’s functional needs, goals, and medical condition.
  • Provide therapeutic exercises, functional mobility training, gait training, balance activities, strengthening, and other appropriate interventions.
  • Assist residents in improving mobility, strength, coordination, endurance, balance, and activities of daily living.
  • Provide treatment to residents recovering from illness, injury, surgery, hospitalization, or other medical conditions.
  • Monitor resident progress and modify treatment plans as clinically appropriate.
  • Document evaluations, treatments, progress notes, and discharge information accurately and in a timely manner.
  • Collaborate with physicians, nurses, occupational therapists, speech-language pathologists, nursing assistants, and other members of the interdisciplinary care team.
  • Participate in care planning and interdisciplinary meetings as required.
  • Educate residents, families, and caregivers regarding therapeutic exercises, mobility techniques, safety precautions, and home exercise programs.
  • Recommend appropriate assistive devices and provide training in their safe use when indicated.
  • Promote fall prevention, safe mobility, and functional independence.
  • Follow facility policies, infection-control procedures, safety standards, and applicable professional practice guidelines.
  • Maintain appropriate communication with nursing and other clinical staff regarding resident progress and changes in condition.
  • Support a resident-centered approach that respects each individual’s preferences, dignity, safety, and quality of life.

Requirements

  • Current and active Physical Therapist (PT) license in New Hampshire.
  • Graduate of an accredited Physical Therapy program.
  • Previous experience in Skilled Nursing Facility (SNF), Long-Term Care, Rehabilitation, or a similar setting preferred.
  • Strong knowledge of physical therapy evaluation, treatment planning, therapeutic exercise, gait training, and functional mobility.
  • Excellent clinical assessment, critical-thinking, and decision-making skills.
  • Ability to work independently while collaborating effectively with an interdisciplinary healthcare team.
  • Strong communication and interpersonal skills.
  • Ability to manage multiple residents and competing clinical priorities.
  • Current BLS certification.

Schedule

36 hours per week.
