About the role
Evaluate, design, and implement a patient’s plan of care; participate in local community service.
Responsibilities
- Manage and monitor full patient cycle: perform an initial evaluation, establish a treatment plan, progress treatment and goals, communicate to physician, discharge, and follow-up.
- Accompany patients during their physician visits.
- Mentor new clinic staff.
- Complete patient documentation in a timely manner including but not limited to, initial evaluations, progress notes, discharge summaries, daily S.O.A.P. notes, functional status reports, functional capacity evaluations and insurance/plan of care forms as necessary.
- Provide local community service such as hosting a booth, presenting in-services, provide coverage at special events, perform complimentary screenings, etc.
- Establish and develop professional relationships with Athletico’s referral sources including, but not limited to, physicians, PAs, ARNPs, chiros, case managers, etc.
Qualifications
- Degreed from an accredited Physical Therapy Program.
- 1-2 years of Athletico PT experience.
- Certified/Licensed: Active clinician license or compact privileges in the state where services will be provided prior to beginning assignments.
