Responsibilities
- Provide physical therapy services to patients in a home health setting.
- Develop and implement individualized treatment plans based on patient assessment.
- Document progress and communicate with referring physicians and other healthcare providers.
- Ensure compliance with all regulatory requirements and insurance guidelines.
Requirements
- Active California Physical Therapist (PT) License.
- Minimum 1 year of Home Health Physical Therapist experience.
- Valid California Driver's License.
- BLS certification preferred.
Qualifications
- Graduation from an accredited Physical Therapy program.
- Current CPR certification.
