The PT will provide skilled therapy services in patients' homes, helping them improve mobility, independence, and overall quality of life while working alongside an interdisciplinary home health team.

- Active PA PT License
- Minimum 1 year of Home Health experience required
- HCHB (Homecare Homebase) experience required
- OASIS experience required
- Reliable transportation
