TalentBurst, Inc is seeking a travel Physical Therapist for a contract position in North Princeton, NJ.
About the role
Penn Medicine Home Health is seeking an experienced Physical Therapist (PT) to provide high-quality rehabilitative care to patients in their homes throughout the North Princeton, NJ coverage area. The ideal candidate will have recent home health experience, be proficient with EPIC EMR and OASIS documentation, and be comfortable managing both Start of Care (SOC) and revisit patient appointments.
Responsibilities
- Evaluate and treat patients in their homes to improve mobility, strength, balance, and functional independence.
- Perform comprehensive Start of Care (SOC) assessments and follow-up (revisit) therapy sessions.
- Complete accurate OASIS assessments and required clinical documentation.
- Develop and implement individualized treatment plans based on patient needs and physician orders.
- Educate patients and caregivers on therapeutic exercises, mobility techniques, fall prevention, and home safety.
- Collaborate with physicians, nurses, occupational therapists, speech therapists, and other interdisciplinary team members.
- Document all patient care accurately and timely in EPIC EMR.
- Monitor patient progress and modify treatment plans as appropriate.
- Ensure compliance with CMS regulations and Penn Medicine Home Health policies.
Requirements
- Active Physical Therapist (PT) license in New Jersey.
- Recent and relevant Home Health Physical Therapy experience is required.
- Experience with Start of Care (SOC) and revisit home health visits.
- Proficiency with EPIC EMR.
- OASIS documentation experience is required.
- Current BLS Certification (AHA preferred).
- Valid driver’s license, reliable transportation, and auto insurance.
- Strong clinical assessment, communication, and documentation skills.
