Travel Physical Therapist - $2,766 per week

About the role

Wellspring Nurse Source is seeking a travel Skilled Nursing Facility (SNF) Physical Therapist for a 14-week assignment starting immediately in Claremont, New Hampshire.

Responsibilities

• Provide physical therapy services to patients in a skilled nursing facility setting.
• Develop and implement individualized treatment plans based on patient needs.
• Assess patient progress and modify treatment plans as necessary.
• Collaborate with interdisciplinary teams to ensure comprehensive care.
• Document patient care activities accurately and timely.

Requirements

• Current licensure as a Physical Therapist in the state of New Hampshire.
• At least one year of recent clinical experience in a SNF setting preferred.
• Bachelor's degree in Physical Therapy from an accredited program.
• Passing scores on the National Physical Therapy Examination (NPTE).

Qualifications

• Excellent communication and interpersonal skills.
• Ability to work independently and as part of a team.
• Strong organizational and time management skills.
• Valid driver’s license and reliable transportation.

Skills

• Proficient in using electronic health records (EHRs).
• Knowledge of Medicare and Medicaid regulations.
• Experience with fall prevention and rehabilitation techniques.

Pay

The pay package is based on 8-hour shifts and 40.0 hours per week (subject to confirmation), with a tax-free stipend to be determined.
