About the role

We are seeking a travel Skilled Nursing Facility (SNF) Physical Therapist for a position in Edgefield, South Carolina. This role offers a 36-hour guarantee on an 8-hour day shift, with occasional weekend call as needed (discussed prior to scheduling). The facility is a 120-bed SNF/LTC with a mix of short-term rehab (STR) and long-term care (LTC) patients. The caseload typically ranges from 7-9 patients but may be higher depending on demand. The facility has a low re-hospitalization rate and strong interdisciplinary team (IDT) communication, including a Nurse Practitioner.

While the general rehab hours are 7 am to 7 pm as a courtesy to residents, schedules can be flexible (e.g., 9 am to 5 pm or 10 am to 6 pm). Candidates open to working every weekend may have a creative schedule, such as Tuesday to Saturday, but must work 5 days per week. The facility is in a rural area but close to Augusta, GA; Aiken, SC; and Greenwood, SC.

Responsibilities

• Provide physical therapy services to SNF/LTC patients, including evaluations, treatment planning, and interventions.
• Manage a caseload of 7-9+ patients, with a mix of short-term rehab and long-term care needs.
• Collaborate with the interdisciplinary team (IDT), including a Nurse Practitioner, to address patient needs and changes.
• Document patient care using NetHealth for rehab and PCC (Point Click Care) for orders, medical diagnoses, and other information.
• Occasionally work weekends as needed (discussed prior to scheduling).

Requirements

• Active Physical Therapist state or compact license.
• Basic Life Support (BLS) certification.
• Minimum of 6 months to 1 year of clinical experience; preference for LTC or SNF experience, but open to discussing other relevant clinical backgrounds.

Skills

• Strong communication and collaboration with interdisciplinary teams.
• Proficiency in EMR/charting systems (NetHealth, PCC).
• Ability to manage a variable caseload in a fast-paced SNF/LTC environment.
• Flexibility in scheduling and willingness to adapt to facility needs.

Pay

Pay package is based on 8-hour shifts and 40 hours per week (subject to confirmation) with a tax-free stipend amount to be determined.
