JD: Physical Therapist (Home Health)

Seeking an experienced Physical Therapist (PT) to provide skilled home health services to adult and geriatric patients. The PT will assess patients, develop treatment plans, improve functional mobility, and help patients achieve maximum independence in their home environment.

Key Responsibilities

• Perform PT evaluations and reassessments
• Develop individualized treatment plans and goals
• Provide gait, balance, strength, and mobility training
• Complete OASIS and home health documentation accurately
• Assess home safety and identify environmental barriers
• Maintain monitoring of pain levels, vital signs, and patient progress
• Collaborate with healthcare teams to ensure quality patient care
• Educate patients and caregivers on exercises and safety measures

Required Qualifications

• Active Physical Therapist License
• Minimum 2 years of PT experience
• BLS Certification
• Strong documentation and time-management skills
• Home Health experience preferred

Compensation Package

• Weekly Gross Pay: $2,636
• Hourly Rate: $26/hr
• OT Rate: $39/hr
• Weekly Per Diem: $1,596 (Non-Taxable)
