About the role
Effectively screens, evaluates, and rehabilitates patients with physical functioning disorders and/or wound care. Develops and implements an appropriate plan of care following all regulatory and clinical practice standards. Utilizes standardized assessments and evidence-based practice to support clinical interventions.
Administers various procedures as part of the rehabilitation plan, including but not limited to: manual techniques, ambulation, therapeutic exercises, modalities, use of supportive and assistive devices, and wound care. Assesses patient progress, adjusts treatments accordingly, and documents all activities in the clinical record per regulatory and clinical practice requirements.
Evaluates treatment outcomes for effectiveness and modifies services in relation to patient evaluation. Provides referrals based on outcomes. Offers consultation and counseling to patients, families, caregivers, and other service providers regarding physical disorders.
Communicates effectively with supervisors and health team members regarding patient progress, barriers, and treatment plans. Provides a comprehensive discharge summary of services per regulatory and clinical requirements. Documents all required information in the electronic documentation system and maintains patient records according to company policy.
Accurately records treatment minutes for services provided per regulatory and clinical requirements. Participates in required meetings, including patient care conferences, utilization review meetings, and rehabilitation meetings. Participates in facility and company-required trainings, in-services, and conferences.
Able to obtain necessary DME or supplies per regulatory and clinical practice requirements. Supervises and supports physical therapist assistants, rehabilitation aides, and/or students. Adheres to APTA and state-required practice acts, code of conduct, and ethics during professional practice as a Physical Therapist.
Requirements
- 1 year of Physical Therapist experience (Required)
- Long-Term Care (LTC) or Rehabilitation experience (Required)
- Missouri State License (Required)
- New graduates acceptable with Level II fieldwork experience (not just Level I observation)
