Job Summary

Physical Therapist Outpatient (PT OP) position available in an outpatient setting in Monmouth, Illinois. Shifts include rotating 8-hour days from Monday through Friday, 8:00 AM to 4:30 PM, with some flexibility on start and end times.

Provides outpatient physical therapy services, following shift notes and rotating shifts.

Required Qualifications

• Active Illinois license or confirmation of application process
• BLS Certification
• Minimum of 1 year of experience

Additional Information

• Local rate policy applies to clinicians with permanent addresses within 50 miles
• Drug-free campus policy with negative drug screens required
• Call-off policy allows up to twelve hours every thirteen weeks without penalty
• Orientation includes 12 non-billable hours, with guaranteed hours not applying during orientation
• Responsible for charting in EPIC
