About the role
CenterWell Home Health is seeking a Travel Physical Therapist for a Home Health assignment serving patients throughout the Baxter Village and greater Fort Mill area. This opportunity offers the flexibility of one-on-one patient care while helping patients safely recover and remain independent in the comfort of their own homes.
As part of a collaborative home health team, you'll manage your daily schedule, build meaningful patient relationships, and provide individualized treatment plans that make a lasting impact.

Responsibilities
- Perform comprehensive physical therapy evaluations and reassessments
- Develop individualized treatment plans based on physician orders
- Treat patients with orthopedic, neurological, cardiac, post-surgical, and medically complex diagnoses
- Improve mobility, strength, balance, endurance, and functional independence
- Educate patients and caregivers on home exercise programs and fall prevention
- Complete documentation in a timely manner
- Collaborate with physicians, nurses, case managers, and the interdisciplinary care team
Requirements
- South Carolina PT License Required
- Home Health experience preferred
- HCHB and OASIS experience preferred
- Valid driver's license and reliable transportation required
