Opportunity

• Work 8-hour day shifts from 8:00 AM to 5:00 PM
• 5 days per week schedule with rotating weekends and holidays
• Provide home health physical therapy services across designated zip code coverage areas
• Treat an average of 6.5 patients per day
• Perform Start of Care (SOC), Resumption of Care (ROC), Recertifications, Evaluations, and standard visits
• Common diagnoses include CHF, orthopedic conditions, and pneumonia
• Minimum 1 year of experience required; new graduates not preferred
• Clinical manager or educator available 24/7 for support
• 2-week orientation with preceptor provided
• Dress code: Scrubs or business casual
• Phone and tablet provided
• Required certification: BLS

Pay Package

The pay package is based on 8 hour shifts and 40 hours per week (subject to confirmation) with a tax-free stipend amount to be determined.
