About the role
Noor Inc. is seeking a travel Physical Therapist for a 13-week assignment starting August 3, 2026, in Kendallville, Indiana.
Responsibilities
- Monday through Friday schedule from 8:00 AM to 5:00 PM
- Point system equivalent to 4-6 patient visits daily
- Tablet used to track time and visits
- Business casual or Hunter green scrubs with white or black top required
- Must complete Oasis Assessments and Start of Care cases
- Common cases include elderly falls and fractures
- Charting system: Homecare Homebase (HCHB)
- Required certifications: BLS (AHA)
- BLS certification not required
- Home health organization serving patients across four counties including Noble, Lagrange, Steuben, and DeKalb
- Focus on elderly care and post-injury recovery
- Facility utilizes Homecare Homebase as its charting system
- Location: Noble County, IN
Requirements
- Physical Therapist Specialty

- Home Health RN opportunity
- Home health organization serving patients across four counties including Noble, Lagrange, Steuben, and DeKalb
- Focus on elderly care and post-injury recovery
- Facility utilizes Homecare Homebase as its charting system
- Required certifications: BLS (AHA)
- Physical demands may vary depending on assignment and client-site requirements
- This position requires the ability to perform the essential functions of the role, which may include standing for extended periods, bending, reaching, and lifting up to 50 pounds on an occasional basis, with or without reasonable accommodation

- Hospital may cancel up to 4 shifts per 13-week assignment or 3 shifts for assignments less than 13 weeks
