This position involves providing high-quality physical therapy services in a home health setting. The ideal candidate will have experience in conducting start of care assessments, recertifications, evaluations, and daily patient visits. Key Responsibilities Conduct start of care (SOC) assessments and recertifications for patients. Perform evaluations and develop personalized treatment plans. Provide in-home physical therapy sessions to patients, focusing on various diagnoses, including CHF, ortho, and pneumonia. Complete daily visits as needed. Work Schedule This position requires working Monday through Friday from 8 AM to 5 PM, with rotating weekend shifts. Participation in holiday rotation is also expected. Qualifications Current BLS Certification required. Minimum of 1 year of experience preferred; new graduates are not preferred. Ability to handle a caseload of 6.5 patients per day. Coverage Areas Candidates will be responsible for providing physical therapy services within the home health environment. Support and Resources Our team members will have access to a clinical manager or educator for support during normal business hours, as well as emergency assistance 24/7 while in the field. Orientation and Training New hires will receive two weeks of orientation, which includes working closely with a preceptor to familiarize themselves with procedures and patient interactions. Attire Scrubs or business casual attire is acceptable for this role.
