About the role

Licensed outpatient physical therapist to provide patient evaluations, develop individualized treatment plans, implement therapy services, educate patients and families, and ensure safe and effective care in an outpatient setting.

Certifications

• Current CPR certification
• Active and unencumbered license for the state where work will be performed

Education Requirements

• Degree in Physical Therapy from an Accredited School

Experience Requirements

• 6 months to 1+ year of outpatient clinical experience preferred

Responsibilities

• Conduct comprehensive evaluations
• Develop and modify individualized treatment plans
• Implement therapy services per plan of care
• Document clinical notes accurately and timely
• Ensure care complies with physician orders and insurance approvals
• Educate patients and families on therapy goals and home programs

Additional Information

• Participation in student programs may be required
• Adherence to state practice acts

• Float rotation may be required
