Responsibilities

• Assess referred patients requiring rehabilitation services, providing direct patient care to evaluate medical condition, functional capabilities, limitations, restrictions, and rehabilitation potential.
• Establish and administer a treatment program with specific goals based on patient capacity and tolerance under physician direction.
• Set realistic and achievable goals for patients, document and record patient condition, and educate patients and families on appropriate physical therapy methods.
• Develop a plan of care for each physical therapy patient.
• Provide skilled physical therapy services/interventions in accordance with physician orders.
• Assess patient needs, plan, evaluate, and modify care to meet goals of physical therapy interventions.
• Collaborate with all disciplines to plan and evaluate team goals for each patient.
