Pioneer Healthcare Services is seeking a travel Skilled Nursing Facility (SNF) Physical Therapist for a travel job in Centralia, Washington.
Responsibilities
- Assess patients' physical abilities and develop personalized treatment plans
- Provide therapy to improve mobility, strength, and functional independence
- Collaborate with healthcare teams to ensure comprehensive patient care
- Educate patients and families on exercise programs and injury prevention
- Monitor and document patient progress and adjust treatment plans as needed
- Implement interventions in various settings
- Participate in discharge planning and coordinate follow-up care for continued rehabilitation
Qualifications
- Doctorate in Physical Therapy
- WA Physical Therapist PT license
- Current BLS/CPR Certification
Pay
$1,991.37 - $2,052.96 / week
Schedule
40 hours per week, 8-hour day shifts
