About the role

The Wound Care Nurse is responsible to assess, plan, implement and evaluate wound care for outpatients. The Wound Care Nurse is involved in the chronic and rehabilitative care of persons with wounds. This position reports to the Program Manager and operates within established organizational and departmental policies and procedures.

Responsibilities

  • Assessing and monitoring wounds
  • Measuring, cleaning and bandaging wounds
  • Working collaboratively with other members of the patient care team to determine if additional treatments or changes in care are required
  • Educating patients and caretakers on wound care, infection and injury prevention, and pressure ulcer care
  • Coordinate DME and VNA services

Schedule

  • Duration: 18 weeks
  • 40 hours per week
  • Shift: 8 hours, days
  • Monday through Friday: 7:00am to 3:30pm
  • Outpatient clinic hours are 7:00am-3:30pm; no rotating shifts required

Qualifications

  • Active MA RN License
  • AHA BLS Certification
  • Minimum 1 year of experience required
  • Certification in Wound Care Nursing strongly preferred

Unit details

  • Unit Name: Outpatient Wound Clinic - RN
  • Clinician to Patient Ratio: Total clinic volume 20-35 patients per day (with 3-4 nurses and 1-2 providers)
  • Patient Population: Chronic patients—treatment of wounds that are difficult to heal. We offer the most current and evidence-based treatments in wound care.
  • EMR (Charting System): Epic
