Requirements

• Active Michigan Physical Therapist License (as per LARA)
• Active American Heart Association BLS certification
• PT (Physical Therapy) Experience
• Outpatient Experience
• Work History
• Skills Checklist, current to 1 year, pertinent to Physical Therapy
• 2 Supervisory Reference Forms completed within the last two years
• Candidates' permanent address must be outside a 100-mile radius from the facility (local candidates not considered)
