Responsibilities

• Completes comprehensive patient evaluations
• Develops and modifies treatment plans

Required Qualifications

• Degree in Physical Therapy from an accredited school
• 1+ year of outpatient clinical experience
• CPR certification
• Active applicable license for Indiana
• No new graduate required

Preferred Qualifications

• Prior assignment with the agency or organization highly desired
• Previous assignment with the organization nice to have
