Assignment Details

  • Serving West Portland, OR and surrounding areas
  • Day shift schedule, 8 hours per day, 5 days per week (8:00–16:30)
  • No weekend requirement
  • No on-call requirement
  • 1:1 patient ratio with up to 6 patient visits per day; standard productivity expectation is 30 points per day
  • Charting via Epic Remote Client for Home Health (laptop) and Rover phone app; OASIS documentation also required
  • Epic EMR experience required
  • Shifts may vary due to system grouping clients to minimize drive time, with occasional maximum radius of roughly 60 miles (60–90 minutes total drive time) per day

Requirements

  • Active Oregon COTA license required if applicable
  • NBCOT certification required
  • Current BLS certification required
  • Minimum 1 year of skilled home health experience required
  • OASIS documentation experience required
  • Reliable personal transportation required
  • Full SSN and DOB required for consideration
  • Minimum of 2 supervisory references covering 1 year of employment within the last 3 years in a comparable specialty/setting required for consideration

Additional Information

  • Services include ADLs, adaptive equipment, home assessments, community re-entry, pre-discharge planning, wheelchair evaluation and positioning, and patient/family education
  • Patient populations may include orthopedic, neurological, and other adult diagnoses such as CVA, TBI, Parkinson's, dementia, amputee, spinal cord injury, and more
  • Business casual attire required; scrubs are allowed but not required and no specific color is mandated — jeans are not permitted
  • First-time travelers are welcome to apply
  • Candidates residing within 50–75 miles of the facility are considered local and may be accepted at the same rate
  • Travelers are expected to float between all facilities within the state; refusal to float may result in termination of assignment
  • Former permanent staff must wait at least 3 months before returning as a traveler, and must not reside within 50 miles of the submitting facility
  • RTO is subject to hiring manager approval; minimal RTO is strongly preferred
  • Primary COVID vaccination series plus one booster required
  • Candidates who have previously worked at a Providence facility in any capacity must disclose the location and dates of prior employment
  • Pay package is based on 8 hour shifts and 40 hours per week (subject to confirmation) with tax-free stipend amount to be determined
