Role & Responsibilities

• Manage treatment plans in a Skilled Nursing Facility (SNF) setting to promote positive patient outcomes across diverse diagnoses.
• Provide therapy services, assess patient needs, and collaborate with healthcare teams to ensure effective care delivery.
• Maintain documentation, adhere to clinical standards, and contribute to patient recovery and well-being.

Qualifications & Skills

• Current BLS (AHA preferred)
• Active state license
• Supervisory professional references
• Successful completion of drug and background screens
• One year of recent SNF experience preferred
• Strong clinical assessment, communication, and teamwork skills are essential
