Responsibilities

• Assess and implement adaptive equipment for patients
• Conduct home assessments and develop care plans
• Perform cognitive and discharge evaluations
• Provide patient/family education and reassessments
• Ensure safety and injury prevention measures
• Work closely with interdisciplinary teams

Requirements

• At least 1 year of experience in adult rehabilitation
• Current certifications: Adaptive Equipment Assessment (Adult), Home Assessment, Adaptive Equipment, Adult Rehabilitation Facility, Amputee, Assessment, Cognitive Assessment, CVA Rehabilitation, Discharge, Evaluation, Lifting and transfer, Patient/Family Education, Plan of Care, Pre-Discharge Planning, Reassessments, Safety and injury prevention
• BLS certification required
