About the Role

We are seeking a dedicated Home Health Occupational Therapist to provide therapy services to clients in their homes in Medford, OR and surrounding areas. This is a 13-week contract position with a day shift schedule of 5x8-hour days (08:00 - 16:30).

Responsibilities

• Conduct evaluations and develop individualized treatment plans.
• Provide therapeutic interventions in accordance with the plan of care.
• Educate clients and their families on treatment plans and therapeutic exercises.
• Document patient progress in compliance with agency policies and procedures.
• Manage a caseload of up to 6 patients per day while meeting productivity standards.

Requirements

• 1 year of Skilled Home Health Experience is required.
• BCLS certification is required.
• OASIS experience is required.
• Reliable transportation is necessary for travel between patient homes.
• Primary series COVID vaccination and one booster are required.

Pay

Pay package is based on 8-hour shifts and 40 hours per week (subject to confirmation) with a tax-free stipend amount to be determined. Contract and local rates may vary based on location and applicant residency.
