About the role

This travel opportunity starts 09/14/2026 for 17 weeks in a skilled nursing facility. Weekly pay ranges from $2,253 to $2,426 per week with guaranteed hours of 35 per week. The shift is a day schedule, 5 days per week, from 8:00 a.m. to 4:30 p.m. The assignment setting is a skilled nursing facility, and the position is a travel - temporary assignment supported by Healthforce Healthcare LLC.

Responsibilities

  • Conduct thorough OT evaluations for adult residents in a skilled nursing facility, review referrals, identify mobility, ADL, cognitive, and upper-extremity function needs, and establish measurable goals
  • Plan and implement individualized treatment interventions aimed at improving activities of daily living, mobility, dexterity, cognition, sensory processing, and adaptive equipment recommendations
  • Reassess resident progress regularly, document treatment plans and progress notes in the medical record, and adjust goals in collaboration with the care team
  • Educate residents and families on safety strategies, energy conservation, activity modification, and use of adaptive equipment to support independence in daily routines
  • Collaborate with a multidisciplinary team including physical therapy, speech-language pathology, nursing, and social work to coordinate care and support effective discharge planning
  • Ensure compliance with regulatory standards and facility policies, maintain accurate documentation, and participate in quality improvement initiatives when applicable

Requirements

  • Active Occupational Therapist license in New Hampshire or eligibility for licensure by endorsement
  • Experience or demonstrated comfort providing OT services to adults in long-term care or skilled nursing settings
  • Strong assessment, treatment planning, and documentation skills; proficiency with electronic health records or equivalent systems
  • Excellent communication and collaboration skills; ability to work effectively within a multidisciplinary team and with residents and families
  • Ability to meet the assignment schedule and work in a skilled nursing facility; willingness to complete required background checks and comply with facility policies
