Job Summary
Occupational Therapist Home Health (OT HH) Greeley, Colorado. This position involves evaluating, planning, and supervising Occupational Therapy treatments for patients in a home health setting. The therapist will work closely with patients and their families to develop individualized treatment plans that promote functional ability and facilitate discharge planning, ensuring care aligns with regulatory standards and organizational policies.
Job Details
- Schedule: Days, 8 hours shift, minimum of 40 hours per week
Required Qualifications
- Must have successfully completed an educational program in occupational therapy approved by the appropriate governing board
- Possess a current Occupational Therapy license from the State of Colorado
- Maintain BLS (Basic Life Support) certification
Preferred Qualifications
- Additional related education and/or experience are preferred
Additional Information
- The position requires the ability to evaluate patients' functional abilities and limitations, establish treatment goals, provide skilled treatment in a safe manner, and involve patients and families in decision-making
- Documentation must be completed within established time frames and reflect comprehensive assessments of patient progress
- The therapist will collaborate with interdisciplinary teams, communicate effectively with patients, families, and other healthcare professionals, and participate in department process improvements
- The role involves providing direct patient care in a home environment, with physical demands including standing, walking, bending, and the potential to lift up to 75 pounds
