Med Ninjas is seeking a travel nurse RN Med Surg for a travel nursing job in Fairfield, California.Job Description & RequirementsSpecialty: Med SurgDiscipline: RNStart Date: ASAPDuration: 13 weeks40 hours per weekShift: 8 hours, daysEmployment Type: TravelJob Summary (List Format): - Provide direct clinical social work services, counseling, discharge planning, and case management to hospital patients and their families. - Assess and document patients’ emotional, social, financial, environmental, and follow-up referral needs. - Collaborate with interdisciplinary healthcare teams to develop and implement individualized psychosocial care plans. - Develop and coordinate discharge plans for assigned caseload, ensuring integration with community-based resources. - Monitor patients’ progress toward care goals, addressing barriers to discharge and estimated length of stay. - Serve as patient advocate, delivering integrated care focused on mind, body, and spirit. - Interpret and communicate social, psychological, emotional, and financial issues to physicians and healthcare team members. - Complete required documentation in patient records and hospital systems (e.g., Cerner, MIDAS+). - Support hospital initiatives such as quality improvement, clinical research, and community networking. - Work effectively with diverse populations, including all ages, cultures, ethnicities, and genders. - Maintain professional skills and comply with hospital and departmental policies and procedures. - Participate in interdisciplinary team meetings and quality monitoring processes. - Perform other duties as assigned to support optimal patient and family care. Skills and Qualifications: - Master of Social Work (MSW) from an accredited school required. - ASW or higher license required; LCSW preferred. - Acute care experience required. - Strong psychosocial assessment, critical thinking, and clinical decision-making skills. - Skilled in grief, loss, and crisis counseling. - Excellent organizational, written, and oral communication skills. - Proficient with word processing; database experience preferred. - Ability to work collaboratively with diverse teams and populations. - Transportation required for hospital coverage as needed. Schedule: - Full-time, 8 hours per day, 40 hours per week; days, some weekends; 8-week initial assignment. Location: - NorthBay or Vacavalley hospitals (assignment based on daily need).Med Ninjas Job ID #17908.
