Role and Responsibilities

• Provide direct patient care in a home setting, including conducting comprehensive assessments, developing and implementing personalized care plans, and performing clinical procedures such as wound care, medication administration, and IV therapy.
• Collaborate with healthcare professionals to ensure coordinated care, monitor patient progress, and adjust treatment plans as needed.
• Educate patients and families on disease management, treatment options, and self-care techniques to promote health and independence.
• Carefully coordinate care with community resources and support services to ensure seamless patient transitions.
