Position Duties
- Coordinate patient care from admission through discharge, ensuring treatments, services, and resources align with each patient's medical needs and recovery goals.
- Collaborate closely with physicians, nurses, and external providers to remove barriers to care, streamline transitions, and support safe, efficient discharge planning.
Minimum Required Qualifications
- MA RN license
- AHA BLS
- 10+ years acute hospital experience in specialty
- Skilled experience in Case Management, Utilization Management, and Discharge Planning
PREFERRED QUALIFICATIONS
- CCM
