Certification Details

• BLS certification required (AHA).
• ACLS certification required (AHA).
• NIH Certification required and must be updated yearly or every 2 years depending on group.

• Must have current nursing license with Nursys verification within 30 days prior to start.

Job Requirements

• Completion of Ascension Pharmacology Exam prior to start, updated every 3 years.
• Completion of Ascension Medical Surgical Exam prior to start, updated every 3 years.
• Completion of Ascension Core Competencies including HIPAA, Joint Commission, NPSG, OSHA topics within a year of start, updated every 3 years.
• Completion of Ascension EKG Exam with minimum 80% score, updated every 3 years.
• Completion of Ascension Unit Specific Checklist (optional for onboarding).
• Completion of Ascension Onboarding Requirements Education Attestation.
• Completion of Ascension Nursing Contract Worker Onboarding onsite.
• Submission of Ascension Badge Photo (forward facing headshot).
• Documentation of two MMR vaccines or positive IGG Titer; no declinations accepted.
• Fit Test required, preferably prior to start, updated annually.
• Documentation of Hepatitis B vaccination series completion or signed declination.
• Influenza vaccine recommended during flu season; exemptions accepted with mask requirement.
• Positive PPD requires negative chest X-ray and annual TB questionnaire.
• Statement of good health or completed physical exam prior to first assignment, updated annually.
• Driver's License/State ID must be current and match address requirements.
• 2 Step PPD or IGRA testing required prior to start; TB questionnaire updated annually.
• Documentation of Varicella immunity or vaccination; no declinations accepted.
• Tdap vaccination valid within 10 years; no declinations accepted.
• Submission of Ascension Profile (resume).
• Two professional references required prior to first assignment.
• Completion of criminal background check including SSN trace, OIG search, and national sex offender registry within 90 days of assignment.
• Completion of EPLS within 90 days of start.
• Completion of Ascension Skills Checklist within one year of start; updated annually.
• 12 Panel Drug Screen within 30 days prior to start; updated annually.
• Verification of education relevant to clinical work within 90 days of start.
• Confirmation of assignment documentation upon initial assignment and updates upon extension.
• Employment verification for past seven years; updated annually.
