Job Requirements
- Documentation of two measles vaccines, two mumps vaccines, and one rubella vaccine or two MMR vaccines, or proof of immunity via blood test.
- Documentation of TB skin test screening within 3 months of start date and updated if >6 month break, or negative QuantiFERON TB Test or T-spot within 3 months of start date and updated if >6 month break.
- Physician Statement signed by MD/DO/NP/PA within 365 days of start date and updated if >6 month break.
- MGB Criminal Background Check within 45 days prior to start date and updated if >6 month break.
- MGB Massachusetts CORI background check within 30 days prior to start date and updated if >6 month break.
- MGB Resume including past 7 years of work experience with city & state.
- Two references within 4 years of submission, minimum 1 supervisor or above in skillset.
- MGB Skills Checklist completed within 1 year of submission.
- MGB Confirmation of Assignment/POI Form required for all initial assignments and extensions.
- MGB Education Verification through background check company verifying highest level of education.
- MGB SSN Trace with addresses/aliases for past 7 years.
- MGB National Sex Offender Check within 30 days of start and updated annually.
- MGB Exclusion Checks within 30 days of start and annually.
- MGB Candidate Submission Coversheet required for submission to initial assignment and new facility.
