Job Requirements
- Licenses/Certificates: License/Certificate Verification required within 30 days of start date (Massachusetts state licensure)
- Medical Exams: Fit Test (N95) within 365 days of start date and updated annually
- Medical Documentation: Respirator Medical Clearance Attestation required if fit test is required, Documentation of MMR vaccination or immunity required, TB Test documentation within 3 months of start date and updated if break >6 months, Health Statement signed by MD/DO/NP/PA within 365 days of start date and updated if >6 month break
- Criminal Background Check: Criminal Background Check within 45 days prior to start date and updated if >6 month break, Massachusetts CORI background check within 30 days prior to start date and updated if >6 month break
- Other: Resume including past 7 years of work experience required for submission, Two references within 4 years required for submission, minimum one supervisor or above in skillset
Skills
- Skills Checklist must be completed within 1 year of submission

Education

- SSN Trace must be run with criminal check

Schedule
Weekend shifts only, 12-hour shifts
Additional Details
- No religious or medical exemptions allowed for Fit Test
- Fit Test updates can be done at Martha's Vineyard & Nantucket if traveler extends beyond original assignment
- Positive PPD requires negative chest X-ray documentation (optional)
- TB Symptom Survey required if Positive PPD (optional)
- Name Change Documentation required if credential documentation does not match legal name
