Certification Details

• American Heart Association BLS certification required for all direct patient care personnel.

Job Requirements

• Fit Test required.
• Trinity 10 Panel Drug Screen required within 30 days of start date.
• Completion of Trinity Code of Conduct Form.
• Completion of Trinity I9 form.
• Completion of Trinity Confidentiality of Patient and Proprietary Information and Information System Security Agreement.
• Completion of Trinity Integrity and Audit Services Form.
• Trinity Respirator Evaluation with physician signature required.
• Proof of COVID-19 vaccine or approved declination/exemption.
• Proof of MMR vaccination or IgG titer.
• Hepatitis B vaccination series required prior to start.
• Influenza vaccine proof or declination (optional).
• 2 Step PPD OR IGRA (Quantiferon/Tspot) required.
• Proof of Varicella vaccination or IgG titer.
• Physical/Health Statement from exam within last year.
• Trinity Profile including resume, skills checklist, and professional references.
• Trinity Conflict Of Interest Disclosure Form.
• Trinity Criminal Background Check within last 30 days.
• Trinity Education Verification.
• Trinity EPLS report within last 30 days.
• Trinity GSA/OIG report within last 30 days.
• Trinity Joint Commission Testing (CORE Exam within a year).
