About the role

A Medical Social Worker is needed in Portland, OR to provide skilled social services to patients under a 1:1 ratio, visiting up to 6 patients daily.

Responsibilities

• Manage patient caseloads with a 1:1 ratio, visiting up to 6 patients daily
• Maintain productivity standards of 30 points
• Provide patient assessments and social support in a home health setting
• Ensure compliance with licensing and certification requirements
• Utilize software for documentation and charting
• Maintain efficient scheduling to optimize drive time and patient visits

Requirements

• Minimum 1 year of home health experience
• OASIS experience
• Licensure in two specific states
• Completed the primary COVID series plus one booster
• BLS certification
• Pharmacology knowledge
• Skills checklist
• Specialty exam

Qualifications

• Clean driving record
• Personal transportation
• No weekend or on-call duties
• Shift: 8 hours during the day
• Business casual attire required
