Role & Responsibilities
Provide case management and patient-centered social work services in a healthcare setting.
Collaborate with healthcare teams to assess patient needs, develop care plans, and ensure appropriate discharge planning.
Maintain accurate documentation and adhere to licensing and certification requirements.
Qualifications & Skills
- Active California license or Compact license
- Current BLS, ACLS, or specialty certifications
- Minimum of 2 years recent case management experience as a Registered Nurse
- Ability to pass background check and drug screening
- Strong communication, organizational, and interpersonal skills
Key Duties
- Conduct patient assessments
- Coordinate services

- Maintain current medical documentation and certifications to support patient care activities
